Oklahoma City, OK – October 6, 2025 – Urban League of Greater Oklahoma City has officially been designated as the  Pathways Community HUB (PCH) for Oklahoma County by the national Pathways Community HUB Institute® (PCHI®).  This designation marks a significant milestone in advancing community-based care coordination and improving health  outcomes across northeast Oklahoma City.

“The Urban League is honored to be designated Oklahoma’s first Pathways Community HUB. This milestone reflects  our commitment to eliminating barriers, strengthening community partnerships, and advancing improvements in health,”  said Urban League of Greater Oklahoma City President, Dr. Valerie Thompson. “Through the HUB model, we will  connect families to vital resources and create pathways to healthier, thriving communities.”

As the first certified Pathways Community HUB in Oklahoma, the Urban League will lead efforts to address the full  spectrum of health-related social needs through a sustainable, data-driven model. The PCH will serve as a centralized  network of community-based organizations, employing and training Community Health Workers (CHWs) to identify risk  factors, remove barriers, and connect residents to essential medical, behavioral, and social services.

As part of its ongoing commitment to improving health outcomes in Oklahoma, Oklahoma Complete Health provided a  $20,000 investment to engage the national Partnership for Community Health Improvement (PCHI®), with the clear  vision of bringing this evidence-based model to the state. This early support enabled critical technical assistance that  helped the Urban League reach a key implementation milestone. This support builds on a broader $300,000, three-year  commitment through the Health Empowerment Zone (HEZ) partnership, which initially seeded the collaboration.  Through the HEZ, Oklahoma Complete Health is advancing community-led initiatives to improve the health of  Oklahomans.

“This project is a testament to what is possible when we prioritize community-driven solutions,” said Oklahoma  Complete Health President and CEO, Clay Franklin. “Together with the Urban League, we are creating a foundation for  lasting impact, ensuring residents have access to the support they need to live their healthiest lives.”

Oklahoma Complete Health and the Urban League are now convening local stakeholders to discuss next steps to  operationalize the HUB.

The Pathways Community HUB model is nationally recognized for its pay-for-performance approach and its use of 21  standardized Pathways to streamline care coordination. By aligning services around whole-person care, the HUB will  help reduce costs, improve outcomes, and promote equity across Oklahoma County.
